Program
Strenght-based individual outdoor treatment program, from 7 to 30+ days long. Continuous assessment, reporting, and supervision. Aftercare and Transition Planning, and Coordination with at-home Professionals.
Follow up
Coordination with At-Home Professionals, and online therapy sessions.

Referral
The process often begins with a referral from a healthcare provider, such as a primary care physician, psychiatrist, or therapist, who recognizes the need for more intensive mental health treatment. Sometimes it is the family who looks for a specific program like ours.

Assessment
Upon referral or family contact, the candidate undergoes a comprehensive assessment by mental health professionals to evaluate their mental health status, symptoms, and treatment needs. This assessment may include interviews, psychological testing, medical evaluations, and review of medical records.

Admission decision
Based on the assessment findings and the individual's treatment needs, a decision is made regarding whether admission to our program is appropriate. Factors considered include the severity of symptoms, risk of harm to self or others, level of functioning, and availability of appropriate treatment resources.

Pre-admission preparation
If admission is deemed necessary and recommendable, the participant and their family or caregivers are provided with information about the admission process, expectations for treatment, and a list of materials is provided what to bring to the program. Participants and their families may also receive guidance on arranging transportation to the programs starting point.

Admission paperwork
Upon arrival, the participant or their legal guardian completes necessary paperwork, such as consent forms, insurance information, and personal information forms. This paperwork ensures that Create Your Change has the necessary information to provide appropriate care and billing.

Orientation
The participant receives an orientation to the program, including an introduction to protocols, its rules and policies, and services offered, and an overview of the daily schedule and routines.

Treatment planning
Shortly before admission, the participant works with a the clinical psychologist and the therapist on the ground to develop a personalized treatment plan based on our assessment findings, treatment goals, and preferences. This plan outlines specific interventions, therapies, medications, and goals for the individual's care during their stay.

Ongoing assessment and review
Throughout the stay at the program, the individual's progress is regularly monitored and assessed by the treatment team. Treatment plans are reviewed and adjusted as needed based on the individual's response to treatment and changing needs.

Discharge planning
As the participant’s symptoms improve and they meet their treatment goals, discharge planning begins to prepare them for transitioning back to the community. This may involve coordinating follow-up care, arranging support services, and providing resources for continued recovery and well-being after discharge.
